The report issued today into the failings of Mid Staffordshire Hospitals NHS Trust (see BBC website) highlights the distress and anxiety suffered by patients and their families. From the Independent Inquiry it has been revealed that there were staff shortages and instances where non-medically trained staff where having to make decisions of a medical nature and in many cases death was a consequence. The report comes only weeks after the David Gray Inquest in which the coroner recorded a verdict of unlawful killing following, Dr Ubani, an out of hours service doctor, administering a lethal injection. Issues of competence and training were raised. We are currently involved with a case arising from a death at Stafford Hospital during the time period looked at by the Independent Inquiry which concerns a death following the non administration of insulin to a diabetic patient. The police have concluded their own investigation into alleged issues of gross negligence amounting to manslaughter and the matter has now been referred back to H M Coroner for the conclusion of the Inquest.
